I/We, the parents or guardians of the above named child, for myself/ourselves and for my/our child, give permission for my/our child to participate in the above mentioned Event on the above written dates.

In consideration of Grow a Generation, LLC agreement to allow my/our child to participate in the above mentioned event, and intending to be legally bound, hereby, I/we agree to indemnify and hold harmless, Grow a Generation, LLC, Cardinal Wuerl North Catholic, the companies and college campuses scheduled on the STEM Careers Tour, including but not limited to RedMorph, Ascender, University of Pittsburgh, Pittsburgh Technology Council, ContainerShip, their agents, successors and legal representatives, against any loss from any and all claims, demands and actions at law or in equity that may hereafter at any time be brought by myself/ourselves, by my/our child, or anyone on his/her behalf, for the purpose of enforcing a claim for damages because of any injury or property damages sustained by my/our child as a result of, or in any way related to his/her participation in the above mentioned event.

   In the event of any injury to my/our child during his/her participation in this event I/we hereby give my/our permission for the necessary medical treatment to be given to my/our child.
    I/We agree that in case of injury to my/our child, I/we will apply my/our hospitalization and/or accident insurance toward payment of the expenses incurred and will not look to Grow a Generation, LLC, their agents, for the payment of any medical costs or injury related costs.

   I grant to Grow a Generation, LLC, Cardinal Wuerl North Catholic, the companies and college campuses scheduled on the STEM Careers Tour, including but not limited to ChemImage, Duquesne University, and Sauereisen the right to take photographs of me and my family in connection with the above-identified event.  I agree that Grow a Generation, LLC may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
